With your hints... Trichomycteridae - no idea the species, but commonly known as the candiru fish.
Without looking at the picture, another guess that would make 'men uncomfortable' would be a barnacle (penis to body size ratio).
Teehee, those are both great guesses, their greatness equally only by their wrongness :-)
I can tell you that it is neither a fish nor a barnacle
